We are seeking a compassionate and dedicated HCA/ Support Worker to join our team in Nottinghamshire and Derbyshire . We are looking for someone who can drive with access to own car to start immediately.

In this role, you will provide essential support to clients at our Supported Living service and community shifts, ensuring their comfort and well-being. You will work closely with the Clinical Mental health Specialist and Care -Co-ordinator to implement care plans and assist with daily living activities. The ideal candidate will possess excellent communication skills and a strong commitment to delivering high-quality care.

Responsibilities

• Assist patients in the community and at the Supported service with daily activities such as bathing, dressing, and personal hygiene.
• Support patients in mobility and transportation, including driving them to appointments if required.
• Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.
• Maintain accurate records of patient care and update care plans as necessary.
• Help with meal preparation and feeding when needed.
• Monitor patients’ health status and report any changes to the nursing staff.
• Ensure a clean and safe environment for all residents by adhering to health and safety regulations.
• Participate in training sessions and continuous professional development.
• Supporting mental health clients with complex OCD, LD, Autism and challenging behaviours.

Qualifications

• Previous Mental Health experience
• Strong command of English language for effective communication with patients and team members.
• Basic IT skills for documentation and record-keeping purposes.
• Ability to drive is advantageous for transporting patients as needed.
• A caring attitude with a passion for helping others.
• Understanding of care plans and the ability to follow them accurately.
• Flexibility to work various shifts, including evenings and weekends as required.
• MAPA trained (Desirable)
• Must be a driver with own car

Mileage paid at 45p per mile for community shifts
